By its execution below please accept this letter as a formal request and authorization to release my energy usage data at the following location(s) to The Energy Professor LLC. This authorization includes requesting billed usage data in the form of kWh, kVA, KW, interval data, and payment history. I also give authorization for The Energy Professor LLC to communicate with a utility REP on behalf of the customer, negotiate one or more service agreements on behalf of the customer for retail electric service, and switch customer from one rep to another rep on behalf of the customer. This historical usage request shall be limited to no more than the most recent 24 month period of previous service.
